From time to time you may need to complete an application or request form when you visit Student Central.

You can download and print the most common forms below, ask our friendly staff in person at one of our Student Centrals, or call Student Central on 0800 10 95 10.

Application for Credit Recognition

Use this form if you want to apply for credit recognition/ cross credit for study at Unitec or another tertiary institution in New Zealand.

If you're applying for cross credits from another tertiary institution in New Zealand you will be charged NZ$105 (incl GST) per application (please note that there is no charge for international students as the cost is included in your course fees).

To submit your application, please deliver in person or post your completed application form (including credit card and/ or cheque details) and all supporting documentation, to one of our cashiers located at Student Central. You can also pay the fee by EFTPOS at any Student Central. Official Academic Record (Transcript)

This is a formal and complete record of programmes studied, results achieved and qualifications awarded while studying at Unitec.

A complimentary Transcript will be included with your graduation certificate (testamur) when you graduate.

If you've misplaced, damaged or would like an additional Transcript, students and graduates can request a Transcript by completing the form below. Return your completed form either in person to one of our cashiers located at Student Central or by email to cashier@unitec.ac.nz.

The standard cost is NZ$25.00 for two copies. Allow 5 working days for processing and 2 - 6 weeks delivery. Priority processing (24 hours) costs NZ$50.00 and can only be done in person at Building 180 Sutdent Central, Mt Albert.

If you studied prior to the year 2000, please note records are sometimes, unfortunately incomplete and take longer to process due to archive access. Please email transcripts@unitec.ac.nz first before ordering to check if a complete transcript is available.

NB: The Transcript reflects your Unitec records at time of processing. Results will not appear if they have not been ratified by your department. If you want your transcript to record your recent or current semester results, please check they are visible in your portal before submitting your request.

Request for Exam Script

You can request a photocopy of any of your exam scripts after they have been marked at no additional cost. Some departments will let you view the exam script - please contact your lecturer if you have any questions.

If you would like to request a copy of an exam script, please complete the Request for Exam Script form and return it to any Student Central.

Request for Letter of Confirmation

If you need a letter to confirm your completion of courses or programmes - for example for an Immigration Visa - please complete the Request for Letter of Confirmation form and return it to any Student Central.

Request for Open Lab Card

Students who are studying ISCG courses with the Department of Computing can request an Open Lab Card for $30 (incl GST). The card gives you access to a computing lab in Building 180 (please note that access hours may vary each semester). To apply for an Open Lab Card, please complete the Request for Open Lab Card form and return it, with payment, to any Student Central.

There is also a swipe card available to students of the Department of Architecture, Landscape Architecture and Design and Contemporary Arts. You will need to pay a $20 bond for the swipe card, which will give you access Building 1 and the surrounding buildings, and the bond will be repaid when the card is returned to Security. Please see the cashier in Building 3 for a swipe card form.

Request for Recount

If you think you've found an error in the addition of the marks on your exam script, you can apply to have script recounted at a cost of $25 (incl GST). A lecturer will check the marking has been added up and correctly calculated. If you would like to apply for a recount on a recent exam script, please complete the Request for Recount form and return it, with payment, to any Student Central.

Please note: This is not a re-marking of your exam script; it is a recount of the marks. You must apply for a recount within 15 days of receiving your result notification.

Request for Replacement Graduation Certificate

Unitec graduates may request a replacement graduation testamur (certificate) for $25 (incl GST). To obtain a replacement testamur (certificate) please complete the request form (including credit card details or cheque) and return it in person to one of our cashiers located at Student Central Waitakere or Te Puna Mt Albert Campus or by email cashier@unitec.ac.nz.

Please allow 10 working days processing time for standard requests and up to 2 weeks for postal delivery within New Zealand, 4-6 weeks for postal delivery to overseas addresses.
If you studied prior to 2000 email transcripts@unitec.ac.nz before ordering to check if a replacement testamur (certificate) is possible. Please note that records for qualifications awarded before 2000 academic year are, unfortunately, sometimes incomplete. We may not, therefore be able to provide a replacement testamur (certificate).Requests for older qualification testamurs (certificates) may take longer than the times advised above.

If your programme is included in either the Spring or Autumn graduation ceremonies, you will need to apply for graduation in order to receive your testamur (certificate).

For programmes not part of graduation the network pathway will distribute your testamur (certificate) to you.

If you require a replacement certificate for a NZQA programme eg New Zealand Certificate in Retail or National Certificate in Real Estate you will need to request this from NZQA.

Special Assessment Circumstances Application

If you were unable to complete a recent assessment, your preparation or performance for the assessment was affected, or you would like to request extra time to complete the assessment, please complete the Special Assessment Circumstances Application and return it to any Student Central.

Note - You will be required to provide a medical certificate or other documentary evidence to support your application.
